APPROVE THERAPEUTIC MASSAGE ENTERPRISE LICENSE FOR HENG MASSAGE, 1250 126TH AVENUE NE, BLAINE, MN 55434
Background
Cuijuan Shen, owner and applicant, has submitted an application for a Therapeutic Massage Enterprise and therapist license for Heng Massage, located 1250 126th Avenue NE, Blaine. The applicant has submitted a completed license application, the license and background fee, along with required documentation.
The Blaine Police Department has completed the background investigation and found nothing that would prohibit the applicant from being issued a license to operate the enterprise. The applicant indicated during her background investigation that she did not intend to perform therapeutic massage herself but has since stated she will be performing massage in addition to another therapist who will apply if the enterprise license is approved and a Certificate of Occupancy is received. The licenses, once eligible, would be valid for the remainder of the 2017 calendar year and eligible for renewal.
Recommendation
By motion, approve a Therapeutic Massage Enterprise License for Heng Massage located 1250 126th Avenue NE contingent upon fire inspection and Certificate of Occupancy.
