Background
Donald Kainz, owner and applicant, has submitted an application for a Therapeutic Massage Enterprise license for Japanese Massage LLC, located 11724 Ulysses Street Suite 120, Blaine. The applicant has submitted a completed license application, the license and background fee, along with required documentation.
The Blaine Police Department background investigation is currently in process on this individual. This license, if approved, would be valid for the remainder of the 2016 calendar year and eligible for renewal immediately.
Recommendation
By motion, approve a Therapeutic Massage Enterprise License for Japanese Massage LLC, located 11724 Ulysses Street Suite 120 contingent upon a successful background investigation, fire inspection, and Certificate of Occupancy.
