Background

The applicant, Dustin Dorr, 2400 110^th Avenue NW, Coon Rapids has submitted an application for an Individual Massage Therapist license to perform massage at Massage Envy, 10450 Baltimore Street NE, Blaine, MN.

The applicant has met the licensing requirements, has submitted the license fee of $75.00 and a background fee of $50.00 with the city, along with the required documentation.

The Blaine Police Department has conducted a background investigation on the applicant and found nothing to prohibit the issuance of this license.

This license will be renewed in the fall and is effective only for the 2013 calendar year.
