APPROVE INDIVIDUAL THERAPEUTIC MASSAGE LICENSE
Background
Staff has received an application from Becky Snyder for an Individual Massage Therapist license. She has submitted all the required documentation, the license fee and a background investigation fee. The Blaine Police Department have conducted a background investigation and found nothing to prohibit issuance of her license. Ms. Snyder will be employed at Salon Mystique, 10950 Club West Pkwy, Suite 150, Blaine.
This license is renewed in the fall and will be effective for the 2013 calendar year.
Recommendation
By motion, approve an Individual Therapeutic Massage license for Becky Snyder.
