APPROVE A VENDING TRUCK LICENSE FOR

BLAINE ANOKA ICE CREAM,

922-94^th AVENUE NE

Background

Faysal Hasan, owner/operator of Blaine Anoka Ice Cream, 922 94^th Avenue NE, Blaine, has submitted an application for a Vending Truck license.  He has deposited a $75 fee and all application materials to operate one vending truck within the City.  The applicant has complied with code requirements relative to liability insurance and has shown proof of a food handler's license through the Minnesota Department of Agriculture, Dairy and Food Inspection Division.  

A background check was completed on Mr. Hasan by the Blaine Police Department, which came back with no issues or concerns on the part of the Police Department.  This vending license shall be effective May 1, 2013 through April 30, 2014.

Recommendation

By motion, approve the vending truck license for Faysal Hasan for Blaine Anoka Ice Cream.
