Background

The applicant, Guangrong Deng, 1029 Paul Parkway, #114, Blaine  has submitted an application for an Individual Massage Therapist license to perform massage at Heng Massage, 1250 126^th Avenue NE, Blaine, MN.

The applicant has met the licensing requirements, has submitted the license fee of $75.00 and a background fee of $50.00 with the city, along with the required documentation.

The Blaine Police Department have conducted a background investigation on the applicant and found nothing to prohibit the issuance of this license.

This license will be renewed in the Fall and is effective only for the 2013 calendar year.
