APPROVE A CARNIVAL LICENSE FOR MAD JAX AMUSEMENTS, 18140 ZANE STREET NW, ELK RIVER  

Background

James Waknitz, owner, of Mad Jax Amusements, has submitted an application to hold a carnival at Church of St. Timothy's from September 7-9, 2012.  

The applicant has deposited with the City a $300 license fee, certificate of liability insurance naming the City as an additional insured, and all other application materials.  

A background check has been completed by the Police Department on the carnival owner, James Waknitz, and no problems or complaints are on file.  An inspection will be completed by the Deputy Fire Marshal during carnival set-up prior to release of the license.

Recommendation

Approve the carnival license for Mad Jax Amusements, 18140 Zane Street NW, Elk River to be held at Church of St. Timothy's, 707 89^th Avenue NE, Blaine, September 7, 8, and 9th, 2012.
