APPROVE THERAPEUTIC MASSAGE ENTERPRISE LICENSE AT PEONY MASSAGE LLC, 11724 ULYSSES STREET, SUITE 120, BLAINE, MN 55434
Background
Jinzhong Wang, current owner of Peony Massage, has sold his Enterprise business to Dongzhou Jiang pending approval of a new Enterprise license. Mr. Jiang has submitted an application for a Therapeutic Massage Enterprise license for Peony Massage LLC, located 11724 Ulysses Street Suite 120, Blaine. The applicant has submitted a completed license application, the license and background fee, along with required documentation. Mr. Jiang applied for the Enterprise license only and the current massage therapists will continue to work at the location.
The Blaine Police Department background investigation is currently in process on this individual and Fire inspection will be completed as part of the Certificate of Occupancy process. This license, if approved, would be valid for the 2016 calendar year and eligible for renewal in the fall.
Recommendation
By motion, approve a Therapeutic Massage Enterprise License for Dongzhou Jiang as Peony Massage LLC, located at 11724 Ulysses Street Suite 120 contingent upon a successful background investigation, fire inspection, and Certificate of Occupancy.
