APPROVE THERAPEUTIC MASSAGE ENTERPRISE LICENSE FOR SERENITY MIND BODY AND SOUL LLC, 11901 CENTRAL AVENUE NE, BLAINE, MN 55434

Background

Salena Cook-Rodriguez, owner and applicant, has submitted an application for a Therapeutic Massage Enterprise license for Serenity Mind Body and Soul LLC, located 11901 Central Avenue NE, Blaine.  The applicant has submitted a completed license application, the license and background fee, along with required documentation.

The Blaine Police Department background investigation is currently in process on this individual and Fire inspection will be completed as part of the Certificate of Occupancy process.  This license, if approved, would be valid for the 2016 calendar year and eligible for renewal in the fall.  This is the City’s 24^th Enterprise license which is the maximum allowed at this time. 

Recommendation

By motion, approve a Therapeutic Massage Enterprise License for Serenity Mind Body and Soul LLC, located 11901 Central Avenue NE contingent upon a successful background investigation, fire inspection, and Certificate of Occupancy.
