APPROVE A TEMPORARY ON-SALE 3.2 MALT BEVERAGE LICENSE AND A CARNIVAL LICENSE FOR MAD JAX AMUSEMENTS, (18140 ZANE STREET NW, ELK RIVER) TO BE HELD AT CHURCH OF ST. TIMOTHY’S, 707 - 89TH AVENUE NE

 

Background

Louis Ochocki, applicant for St. Timothy’s Catholic Church, has filed an application for a Temporary On-Sale 3.2% Malt Beverage license and James Waknitz, owner of Mad Jax Amusements, has submitted an application to hold a carnival at St. Timothy’s Carnival to be held at 707 - 89th Avenue NE for the following dates and times:

 

Ø                     Friday, September 11, 2015        5:00 p.m. - 11:00 p.m.

Ø                     Saturday, September 12, 2015  12:00 p.m. - 10:30 p.m.

Ø                     Sunday, September 13, 2015     11:30 a.m. - 4:30 p.m.

 

Mr. Ochocki has deposited with the City a $50 license fee and a certificate of insurance (liquor liability) covering this event.  The City has been named as an additional insured on the certificate of insurance.

 

Mr. Waknitz has deposited with the city a $300 license fee and a certificate of insurance naming the City of Blaine as an additional insured.

 

Background checks for Mr. Ochocki and Mr. Waknitz have been completed by the Police Department and no problems or complaints are on file.  An inspection will be completed by the Deputy Fire Marshal during carnival set-up prior to release of the Carnival license.

 

Approximately 500 people are expected to attend the event.

 

 

Recommendation

By motion, approve a Temporary On-sale 3.2% Malt Beverage License and a Carnival License for St. Timothy’s Carnival, 707 - 89th Avenue NE, to be held September 11, 12 and 13, 2015.
