Title
APPROVE 2012 THERAPEUTIC MASSAGE ENTERPRISE LICENSE
Background
On April 19, 2012 the City Council approved the new ordinance requiring all massage therapy enterprises be licensed. The ordinance became effective May 19, 2012.
Debra Lynn Johnson, owner of Tiny Apples Massage Therapy, 10299 University Avenue NE has applied for a Massage Therapist Enterprise License. Staff has received the required documentation, a prorated license fee for 2012 and a background investigation fee from each applicant.
The Blaine Police Department have conducted a background investigation and found nothing to prohibit issuance of this license.
This license is renewable in the fall with an effective date of January 1, 2013 and is effective for a calendar year.

By motion, approve a 2012 Therapeutic Massage Enterprise License for Tiny Apples Massage Therapy, 10299 University Avenue, Suite 207
