Background

Yuping Wang, owner and applicant, has submitted an application for a Therapeutic Massage Enterprise license for Indigo Massage, located 1506 125^th  Avenue NE, Blaine.  The applicant has submitted a completed license application, the license and background fee, along with required documentation.  Ms. Wang is requesting the license as she is purchasing an existing location.  

The Blaine Police Department have conducted a background investigation on this individual and found nothing to prohibit issuance of her license.

This license will be valid for the 2015 calendar year and eligible for renewal in the fall.  

Recommendation

By motion, approve a Therapeutic Massage Enterprise License for Indigo Massage, 1506 125^th Avenue NE, Blaine, MN 55449.
