Background

Sai Lian Zhu, applicant, has submitted an application for an Individual Massage Therapist license at Moonlight Massage located at 1617 93^rd Lane NE, Blaine.  The applicant has submitted a completed license application, the license and background fee, along with required documentation.  

The Blaine Police Department has conducted a background investigation on this individual and found nothing to prohibit issuance of her license.

This license is valid for the 2014 calendar year and will be renewed in the fall.  

Recommendation

By motion, approve an Individual Therapeutic Massage license for Sai Lian Zhu.
