Executive Summary
Council is asked to approve a massage enterprise license to Sampson Wellness LLC, dba Elevate Fitness & Massage. This license will allow the applicant to provide massage therapy services at their business location.
Background
Teresa Sampson has submitted an application for a therapeutic massage enterprise license for Sampson Wellness LLC, dba Elevate Fitness & Massage, 10945 Ulysses Street NE #500. The applicant has met the licensing requirements by submitting all necessary forms and paying the associated fees. The Blaine Police Department is in the process of conducting a background investigation on Ms. Sampson. Issuance of the license is contingent upon successful background investigation of all partners.
If approved, the total number of massage enterprises in Blaine will be 10 and the current license cap will be reached. This license, if approved, would be valid for the 2022 calendar year and eligible for renewal in December. Any therapists who will be employed at this new enterprise will be required to obtain separate licensing as well.
